OCCUPANCY TERMINATION DOCUMENT

This document, presented to the spouse, terminates the Alzheimer's patient's occupancy in the Adult Home thereby giving he and his wife 30-days to find a new place to live. 

This was Camphill Ghent's steps #1 and #2 for resolving the "problem" with this patient.
